3 Things Most People Don't Know About Hyperbaric Oxygen Chamber Therapy

Hyperbaric oxygen therapy—HBOT—has a split reputation. To some it's a serious hospital treatment for divers and burn victims; to others it's a celebrity-endorsed anti-aging chamber you can rent by the session. Both pictures are incomplete. Beneath the headlines there are a few facts that genuinely change how you'd think about it. Here are three.
1. It's not really about breathing "more" oxygen—it's about pressure
The intuitive assumption is that HBOT just gives you extra oxygen to breathe. But under normal conditions, your red blood cells are already nearly saturated—breathing harder or richer air doesn't add much. The real trick is pressure.
HBOT works on a principle of physics called Henry's law: the amount of a gas that dissolves into a liquid rises with the pressure of that gas. By having you breathe 100% oxygen inside a chamber pressurized to roughly two to three times normal atmospheric pressure, the therapy forces large amounts of oxygen to dissolve directly into your blood plasma—the fluid part of your blood—rather than relying on hemoglobin. The effect is dramatic: plasma oxygen levels can rise to many times normal, and oxygen transport is no longer limited by your red blood cells at all. That's what lets oxygen reach tissues with poor blood supply, like a stubborn wound or radiation-damaged area, where it otherwise couldn't get. The chamber isn't an oxygen tank; it's a pressure vessel, and the pressure is the active ingredient.
2. It mobilizes your own stem cells—up to eightfold
This is the fact that surprises people most, and it's grounded in solid research rather than wellness marketing. A course of hyperbaric oxygen treatments does more than flood tissues with oxygen—it appears to recruit your body's own repair cells.
In a study led by Stephen Thom at the University of Pennsylvania, a typical course of HBOT increased the number of stem/progenitor cells circulating in patients' blood by roughly eightfold. The mechanism is counterintuitive: the high-oxygen environment boosts production of nitric oxide in the bone marrow, which triggers the release of these progenitor cells into circulation. Thom described it as one of the safest clinical ways to increase stem cell circulation, far safer than the pharmaceutical alternatives. Subsequent laboratory work has explored how HBOT influences stem cell behavior through pathways like mTORC1 and Wnt signaling. These cells are central to healing, which helps explain why a therapy that started as a treatment for decompression sickness keeps showing up in regenerative and wound-healing research.
3. The FDA-approved uses are narrow—and a lot of clinics go well beyond them
Here's the part that matters most before you book a session. HBOT is a legitimate, FDA-cleared treatment—but only for a specific, limited list of conditions where the evidence is strong.
Those approved uses include decompression sickness (the diver's "bends"), carbon monoxide poisoning, gas embolism, certain non-healing diabetic wounds, delayed radiation injury, severe infections like gas gangrene, crush injuries, compromised skin grafts, and severe anemia, among others—around 13 to 14 recognized indications in total. What's striking is how many popular uses fall outside that list. The FDA has specifically noted that some centers promote hyperbaric chambers for conditions that haven't been cleared or approved—naming things like cancer, autism, Lyme disease, and Alzheimer's. Anti-aging and general "wellness" claims sit in the same unproven territory.
This doesn't mean off-label uses are worthless—some are under active research—but it does mean the marketing often runs ahead of the evidence. Sessions aren't cheap (commonly a few hundred dollars each, often across 20 to 40 visits), so it's worth knowing whether you're paying for a proven indication or an experimental one. The FDA also advises getting treatment at a properly accredited facility.

Hyperbaric oxygen therapy is more interesting than either of its reputations suggests. It's fundamentally a pressure treatment, not just an oxygen one; it appears to mobilize your own stem cells in a way that's well-documented; and its proven uses are narrower than the booming wellness market implies.
